This month's issue is dedicated to SurveyWorks and we are reaching out to parents for your feedback. The results of the survey will help us to determine the needs of our school and it is a meaningful way for parents to share their input on a variety of topics pertaining to CHS.
You can take the short survey online or on paper between February 6th and March 31st. To take the survey, visit: surveys.panoramaed.com/ride and enter the access code: 98101family. If you don't have access to the internet, please contact Mrs. Nikki Scott at 364-7778 ext. 2102 to access a paper survey. Responses are anonymous and cannot be linked to any individual; no one can tie your responses back to you.
Topics on the survey include: School Safety, Learning Strategies, School Climate, Learning Behaviors and others. Students will be taking the survey on February 7th and 8th. To see the actual surveys, visit the following site: Surveyworks.ride.ri.gov. All surveys are anonymous and are available in English and Spanish. Accommodations will be offered as necessary.

A Note from the clinic...
The State is requiring that we have a Physical Exam and a Meningococcal
vaccine, received after the age of 16 years old, for every 12th grade student. The
reasoning for this notification is that anyone with an upcoming yearly physical
can easily get a copy of their physical at the time of the office visit and
lessen the burden of our requesting one at a later date. Please be advised that
any current documentation that meets the above criteria may be sent to the
clinic at any time. We also have the ability to contact the Doctor for you if fill out the following form and send it to the clinic: Release of Information Form
